The initial contact and consultation is about listening. We want to hear you and make sure that we understand your circumstances as deeply as we can. This starts when you first make contact with us and continues in the initial assessment.
When you contact us for the first time, we will set up an assessment, which is usually done in person because that is the best way for you to get a sense of who we are and for us to begin to get to know you. However, if circumstances call for it, we can arrange a virtual assessment at your request.
During the assessment, we will visit you or your loved one’s home. In that time together, we will continue to listen and offer any advice you may need. The purpose of this assessment is to get all the information we can about your care needs so that we are well-placed to begin forming the care plan with you.

After the home visit and initial assessment, our clinical team works to create a custom care plan that fits your unique circumstances. The care plan has to meet our criteria of being person-centred and outcome-focused, so it is not something that is merely put together by our clinical team. It is a bespoke plan created with your input and aimed at the outcomes you have in mind.

Once everyone has agreed on the care plan, we will assign a select small group of carers to you. They will provide the personalised support and care that you need.
We understand that the relationships between carer and the person or family being cared for are significant and important, as is the knowledge that the carers build up.
This group is led and co-ordinated by a senior care person or team leader who oversees and monitors the overall plan and care provided.
All the carers have been courteous and professional in all aspects of their work. They have happily done any task I have asked of them, with out any problems. I would recommend the carers that I have had easily.

The next step in the process is getting everyone on and comfortable with the CareCircle App. This is an extremely useful app and important step that gives families access to notes, tasks, visit times, carer details and other useful information. It also has a “real-time updates” facility for connected family members and provides the ability for you to request on-call support. The CareCircle app also integrates with Careline Live so that you can monitor which tasks have been completed and which still need to be done.
Do not worry, we will help everyone get set up and comfortable with the apps. We walk you through it step by step. The important thing for you to know right now is that everything is done transparently and that everyone is always kept fully in the loop.
Of course, we are always available on our dedicated on-call phone number for queries for those who are not comfortable using the apps.

At this point, the care plan will be carried out, and everyone will get comfortable with the routine. However, we never simply set a care plan in motion and forget about it. The situation is constantly monitored, so that adjustments can be made as things evolve.
As we said earlier, each care plan is tailored to the individual and their family because we genuinely care and want what is best for the people for whom we provide care. Because of this, you can expect periodic reviews of the care plan and you will be a part of these reviews.
